Job Description
Your primary role is to understand and interact with your community so that you can exceed the expectations of patients and provide them with care they value.
What success looks like:
- ≥ 45 visits/week
- ≥ 85% pt. retention visits 1-3
- ≥ 80% pt. retention visits 4-6
- ≥ 3.8 units/visit
- 5 patient requests/month
Job Requirements
- Valid Indiana PT license
- Strong communication and interpersonal skills
- BLS Certification preferred
