Physical Therapist (2556), Citywide | San Francisco Department of Public Health | Full-Time | Permanent Civil Service The San Francisco Department of Public Health (SFDPH) protects and promotes the health of all San Franciscans. Physical therapists work in person across SFDPH's hospitals, clinics, and programs, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers. Under general supervision, the Physical Therapist provides physical therapy evaluations and treatments to a diverse client population. Duties include: - Performing physical therapy evaluations, including assessment of functional status and physical condition - Developing treatment plans, including goal setting - Administering physical therapy interventions and procedures, including education and training of caregivers - Communicating with other care team members with professionalism and respect - Documenting per regulatory and Practice Act guidelines - Overseeing care provided by physical therapist assistants in all settings and home health aides in the home - Providing clinical case management and supervision of the home health care team when appropriate - Participating in patient care meetings and performance improvement projects - Explaining treatment objectives to patients and families, including instruction in prosthetic and orthotic devices, special equipment, and home programs - Instructing students and auxiliary personnel, and coordinating rehabilitation with physicians, nurses, teachers, occupational therapists, speech-language pathologists, and other departments This is a continuous citywide exam posting (Recruitment ID CCT-2556-H00079).
