Home Health Physical Therapist (Full-Time Salaried, Not Per Visit)
Monumental Post Acute Care at Home LLC
Bala Cynwyd, PA
Job Description
Family-Owned and Operated Home Health Company
Now Hiring for Philadelphia, Montgomery, Bucks, and Delaware Counties
About Us
We are a family-owned and operated home health company committed to providing compassionate, high-quality care in the comfort of our patients' homes. Our mission is to help patients become as independent as possible, or remain as independent as possible, through personalized, patient-centered services. We also believe that excellent care starts with a supported team, which is why we value work-life balance, flexibility, respect, and open communication.
Position Summary
We treat our clinicians differently than other providers. Our full-time salaried team members are considered an integral part of our team, and are members of the Leadership Team, actively providing feedback and engaging in helping us become the best home health provider in the area. In many cases, our full-time team members receive a base salary and can then achieve steps in pay based on the number of weekly visits completed. Restrictions and rules are laid out in individual contracts.
We are seeking a caring and motivated Home Health Physical Therapist to join our team. In this role, you will provide skilled physical therapy services to patients in their homes, helping them improve mobility, strength, safety, function, and overall independence. - Develop and implement individualized treatment plans focused on improving mobility, balance, strength, endurance, safety, and functional independence.
- Educate patients and caregivers on home exercise programs, fall prevention, safe transfers, equipment use, and strategies to support independence at home.
- Monitor patient progress, update goals as appropriate, and communicate changes in condition to the care team and physician as needed.
- Complete accurate and timely clinical documentation in accordance with company standards and regulatory requirements.
- Collaborate with nurses, occupational therapists, speech therapists, aides, physicians, families, and other members of the interdisciplinary team.
- Supervise Physical Therapist Assistants, when applicable, in accordance with state regulations and company policy.
- Promote a positive patient experience through respectful, compassionate, and professional care.
Qualifications
- Current Physical Therapist license in the state of practice.
- Graduate of an accredited Physical Therapy program.
- Current CPR/BLS certification preferred or required based on company policy.
- Home health experience preferred, but not required for the right candidate.

- Ability to work independently in the home setting while maintaining regular communication with the care team.
- Reliable transportation, valid driver's license, and auto insurance as required for home visits.
- Commitment to delivering high-quality, patient-centered care.
Schedule and Territory
This is a full-time position with flexible scheduling based on patient needs. Clinicians schedule their own visits, so they can create and maintain their own schedules. Visits are completed in patients' homes. We are currently serving Philadelphia, Montgomery, Bucks, and Delaware Counties.
Why Join Our Team?
